Patient Information

Initial Consultation

Carole only accepts 1-2 new patients per month. If you would like to work with Carole, please email her or call her directly to make an appointment or to get on the waiting list.  Please note that if you have not seen Carole in the last year, you are considered a new patient.

Your first visit will be a 1-hour appointment, and will be spent mainly discussing your symptoms and treatment options. Please bring any health examination and medical test results, including X-rays, blood tests, MRI reports. Please wear or bring comfortable, stretchy clothing like leggings, yoga pants or shorts.

Please also bring your completed Patient Intake and Consent Forms.

After the initial appointment, depending on your situation, Carole may refer you to another practitioner, or for testing. If you continue with Carole, she will offer treatment, as well as other instructions including dietary, postural instructions, and additional information. For these treatments, please drink 4 glasses of water in the 4 hours leading up to your appointment so that you are well hydrated.

Dr. Martin does not have a receptionist, so all contact will be made directly with her.
